rsh/ssh patch

   This patch makes it possible to use a user-specified program instead of
   rsh to connect to the IMAP server. Set the shell variable PINE_RSH the
   full path to the program to use (e.g /usr/bin/ssh). If none is specified,
   Pine will by default use rsh, just as before.
